A passport for people in care home settings how have diabetes has been launched to provide carers with vital information about residents' diabetes needs.
The document provides care staff with simple steps to follow in order to manage people's treatment effectively. It is hoped that this will help avoid unnecessary hospital admissions.
The initiative has been devised by charity the InDependent Diabetes Trust, the Institute of Diabetes for Older People (IDOP) and the English Community Care Association (ECCA.)
