Nurses and care staff have developed a self-assessment tool to compare current dementia care with the best practice criteria.
The framework is for health and social care staff, as well as for wider teams and organisations. It can be used when considering changes to improve practice, developing care plans and reviewing progress around their involvement in dementia care across the whole pathway.
The tool was developed by nurses and care staff, to ensure the implementation of the nursing contribution to the 6Cs and dementia pathway.
It will help nurses and care staff evaluate dementia services and what they need to do over the coming year to improve them. It will also allow the sharing of good practice and learning on a wider scale.
The contribution to the dementia care pathway by nurses and care staff includes: increasing basic awareness, reducing stigma, assessing, diagnosing and continuing to support people with dementia and carers through to end of life care.
The self-assessment framework can be used to baseline the current services against best practice criteria, develop a shared action plan with staff groups and then re assess following implementation of action plans to demonstrate continuous improvement.
It provides a self- assessment framework for health and social care staff to use when considering changes to improve practice and to develop a plan and to review progress around their involvement in dementia care across the whole pathway. Plans may be developed by the team working in an area but will be supported by managers and the organisation.
